There is an opportunity to work in the community playing major clinical and care coordination role to improve quality of life and reduce Emergency Department (ED) presentation and hospital admission for people living in the community who are at risk of deteriorating health.

Remuneration: 2,170.30 - $2,241.60 per week

Where you'll be working

The position of Complex Care Coordinator sits within Northern Sydney Home Nursing Service (NSHNS) in the Division of Primary and Community Health Care, Northern Sydney Local Health Service. The care is delivered in the Hornsby Ku-Ring-Gai area. The position is based at Macquarie Hospital, North Ryde.

What You'll Be Doing

The purpose of the Clinical Nurse Specialist (CNS) position is to provide senior nursing clinical leadership in the development of specialist clinical practice and service delivery within a nursing unit, team or service. The CNS employs advanced clinical skills and exercises extended autonomy of decision-making working closely with managers and other senior clinicians.

This position plays a major clinical and care coordination role through the provision of expert clinical assessment, referral and coordination of health and community services for people with severe chronic disease and complex care requirements living in the community who are at risk of deteriorating health that may result in declining quality of life and a hospital admission.

Candidates Will Need To Meet The Following Criteria

  • Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
  • Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
  • Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, rand community organisations.
  • Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
  • Current NSW Drivers License.
