Carer Position

We are seeking a dedicated and empathetic carer to build meaningful relationships and make a positive impact in the lives of our clients.

About the Role:

  • Conduct thorough client assessments and develop tailored care plans that cater to individual needs, ensuring quality care and support.
  • Effectively manage client information through our CRM system, maintaining accurate details and up-to-date case notes.
  • Collaborate with the care team to engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes.
  • Provide person-centered care, supporting clients with daily activities, transportation, and social interactions within their homes.
  • Deliver care services in accordance with individualized Care Plans, promoting independence and well-being.
  • Encourage participation in community-based recreational and social activities, enhancing overall quality of life.

Key Requirements:

  • Certificate III Aged Care or Individual Support, or equivalent qualification.
  • Australian Driver's Licence (minimum Green Provisional) and a roadworthy vehicle for work-related travel.
  • Proficiency in using technology, including smartphones, tablets, and laptops.
  • Ability to meet relevant probity checks and maintain confidentiality.
