Job Description

We are seeking a dedicated Care Navigator to provide coordination and support services to individuals in need.

The ideal candidate will have strong knowledge of community resources and support systems, excellent communication, time management, and organizational skills.

This role requires a Bachelor's degree in Social Work, Psychology, Human Services, or related field, with a Master's degree or prior care navigation experience being preferred.

Key Responsibilities:
  • Conduct comprehensive client assessments and develop individualized service plans
  • Provide referrals and connections to housing, health care, employment, and community resources
  • Advocate on behalf of clients with service providers and agencies
  • Maintain accurate and timely case documentation using electronic health record systems
  • Coordinate with therapists, social workers, and program directors as part of a multidisciplinary team
  • Monitor client progress and adjust service plans as needed
  • Ensure compliance with privacy regulations and agency policies
Requirements:
  • Strong knowledge of community resources and support systems
  • Excellent communication, time management, and organizational skills
  • Bachelor's degree in Social Work, Psychology, Human Services, or related field
  • Prior care navigation experience an asset
