Senior Aged Care Specialist

We are seeking a highly skilled Senior Aged Care Specialist to join our team. This part-time role involves coordinating the provision of high-quality aged care support services to our clients in the local community.
Key Responsibilities:
- Meet and greet with potential clients advising on the services provided and suitability of supporting them or their loved one
- Manage bookings of facilities as clients reserve their stay with us in our Cottage Respite
- Follow up direct enquiries from referral sites, including assessors, DVA and clients directly
- Develop and implement individualised care plans for clients, in collaboration with them and their families
- Coordinate delivery of in-home care and support services, such as personal care, domestic assistance, and social activities
- Liaise with clients, their families, and external service providers to ensure seamless service delivery
- Monitor client wellbeing and adjust care plans as needed
- Maintain accurate and up-to-date client records and documentation to maintain compliance
- Provide support and guidance to care workers, and assist in their professional development
- Contribute to the continuous improvement of our aged care support services
Requirements:
- Relevant qualification in aged care, disability support, or a related field
- Minimum 2 years' experience in a care coordination or case management role
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients and their families
- Strong organisational and time management skills, with the ability to prioritise tasks and work effectively under pressure using technology
- Demonstrated understanding of the aged care and disability support sectors, and the ability to navigate relevant legislation and regulations
- Empathy, patience, and a genuine commitment to improving the lives of older adults and people with disabilities
- A valid driver's license and access to a reliable vehicle
- Knowledge of current Aged Care standards, and an awareness of new changes being implemented
