About The RoleWe are seeking a dedicated practitioner to join our team based in Mackay, focusing on family violence services.
- Provide trauma-informed case management in line with TSA standards and models of care
- Build strong relationships with clients that encourage empowerment, safety, and participation
- Conduct assessments of client needs, risks, and goals from intake to exit
- Develop individualised case plans in collaboration with clients, ensuring their active participation
- Facilitate access to appropriate support services and resources
- Tertiary qualification in Social Work, Welfare or related field
- Demonstrated experience in case management within the family violence sector
- Experience working with vulnerable populations is highly desirable
