">Care Coordinator

This role involves overseeing and enhancing the delivery of in-home aged care services. The successful candidate will be responsible for processing referrals, assisting clients in commencing services, and continuously reviewing and adjusting care plans to suit their evolving needs.

The key responsibilities include undertaking comprehensive client assessments, developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders, ensuring care plans are monitored and reviewed on a regular basis, collaborating effectively with internal stakeholders, developing strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients, and representing the organisation at external forums and meetings.

To be successful in this role, you will need to have demonstrated experience in Aged Care or Nursing case management, a strong understanding of Home Care Packages, Commonwealth Home Support Programme, Aged Care services, and My Aged Care Standards, effective communication and administration skills, and proficiency in utilising Microsoft Office, Outlook, and database systems.
