This role offers high-level case management support to individuals participating in programs, focusing on clients with complex needs and psychosocial requirements.

The role operates within a jurisdictional framework, delivering justice services to the community across various locations.

As an advanced case manager, you will provide comprehensive case management to individuals with complex needs, including support for mental health and disability requirements.

You will be responsible for accessing, coordinating, and monitoring treatments and supports for clients, encompassing housing, alcohol/drug treatment, family violence interventions, and mental health treatment.

Key responsibilities include:

  • Providing expert advice to judicial authorities and stakeholders
  • Accessing and coordinating treatments and supports
  • Identifying client risks and developing risk management strategies
  • Supporting clients with diverse needs and backgrounds
  • Liaising with judicial authorities, legal professionals, and court staff

To succeed in this position, you will require extensive experience in assessing and managing clients with complex mental health and/or disability support needs, as well as experience in mentoring or coaching health and social welfare staff.

Absolutely essential skills and qualifications include:

  • Extensive experience in assessment and case management
  • Mentoring or coaching experience
  • Detailed knowledge of the mental health and/or disability service system
  • Sound knowledge of evidence-informed practice models and frameworks
