Key Responsibilities:

  • Provide advanced case management support to clients with complex needs
  • Assess and manage complex mental health and disability support needs
  • Establish and maintain effective relationships with stakeholders
  • Apply evidenced-informed practice models and frameworks in client work

Required Skills and Qualifications:

  • Extensive experience in assessment and case management of clients with complex mental health and/or disability support needs
  • Mentoring or coaching skills, preferably in health and social welfare settings
  • Detailed knowledge of the mental health and/or disability service system
  • Ability to establish and maintain effective stakeholder relationships
  • Sound knowledge of evidenced-informed practice models and frameworks
