Aged Healthcare Specialist

This role plays a vital part in enhancing access to healthcare for older adults by addressing service gaps in dynamic care models.
The focus is on improving the health of older adults through timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions.
Collaboration with other healthcare team members is essential to provide high-quality patient care.
About The RoleThis position has primary specialization in Aged Care with opportunities for development into Emergency and Acute Care, Mental Health, and Chronic and Complex Care.
Departmental locations may include Inpatient Units, Outpatient Services, In-reach, Outreach, Community, and Emergency.
Key Responsibilities- Develop and implement individualized care plans for older patients.
- Provide education and support to patients and their families on health-related issues.
- Conduct thorough assessments of patients' physical and emotional needs.
- Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes.
- Current registration as a Registered Nurse or eligible for endorsement as a Nurse Practitioner.
- Completion of a Master's degree in Nursing or relevant field.
- Demonstrated experience in Aged Care and/or emergency and acute care settings.
This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
