Job Description:

We are seeking an experienced Nurse Practitioner or Transitional Nurse Practitioner to enhance access to healthcare for older persons by addressing service gaps within dynamic Models of Care (MoC).

The successful candidate will be responsible for improving health outcomes for older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.

The primary metaspecialty is 'Aged Care', with supported development of your Scope of Practice (ScOP) into 'Emergency and Acute Care', 'Mental Health', and 'Chronic and Complex Care'. Departmental locations may include Inpatient Units, Outpatient Services, In-reach, Outreach, Community, and Emergency settings.

Requirements:
  • Current registration as a Registered Nurse or Enrolled Nurse in Australia
  • Completion of a Master's degree in Nursing or related field
  • Endorsement as a Nurse Practitioner or working towards endorsement
  • Provision of proof of Australian citizenship or permanent residency
