The Role

This role contributes to our comprehensive health support services, aiming to achieve improved health outcomes for Aboriginal and Torres Strait Islander people. The successful candidate will be responsible for:

  • Explaining procedures and treatment to clients, ensuring they understand their health management options.
  • Contributing to the effective management of patient care plans, including participating in patient care conferences and managing follow-up procedures.
  • Identifying client care needs and documenting consultations in medical records.
  • Assisting healthcare professionals with communication, treatment, and advice relevant to client care.

Requirements

To be successful in this role, you'll need to have:

  • A Certificate III in Aboriginal Primary Health Care (Clinical Practice) or higher.
  • Previous experience in a relevant field, preferably in Aboriginal health.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team.

Preferred Requirements

It would be advantageous if you have:

  • Minimum two years' experience in a primary health setting.
  • Previous experience working with Aboriginal and Torres Strait Islander clients.
  • Experience in chronic disease management.
