Senior Care Coordinator Position

We are seeking a highly skilled and compassionate Senior Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for developing and implementing personalized care plans for clients, ensuring all services are aligned with their health and wellness goals.

Requirements:

  • Graduate degree from an accredited school of nursing or related field
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Montana
  • Previous experience in healthcare or a related field, with a focus on care coordination or case management
