The successful applicant will be responsible for conducting thorough assessments and person-centred planning to identify the needs of our clients. This will involve working closely with clients, their families, and/or carers to develop meaningful goals and practical strategies that utilise The Positive Behaviour Support Capability Framework. With a commitment to eliminating restrictive practices, you will work tirelessly to create individualised support plans that reflect our clients' passions, strengths, and interests.

Responsibilities
  • Conduct thorough assessments and person-centred planning to identify the needs of our clients
  • Develop and deliver evidence-based support plans within a complex support space
  • Work closely with clients, their families, and/or carers to develop meaningful goals and practical strategies
  • Identify and build the knowledge, capacity, and strengths of those who support our clients - family, carers, friends, etc
Required Skills and Qualifications

To be successful in this role, you will need to possess:

  • Current suitability (i.e. registration) with the NDIS Commission as a Behaviour Support Practitioner
  • Demonstrated ability to work positively in a multidisciplinary team
  • Outstanding communication, influencing, and interpersonal skills
  • Confident computer skills
  • Flawless writing skills
  • Current Driver's Licence and own reliable vehicle
