Job Description:
As a senior care manager, you will oversee the care management of approximately 12-18 clients with complex needs. Your primary responsibility is to provide expert advice and support to your team members and ensure that care practices meet relevant standards.
Key Responsibilities:
- Provide high-quality care management advice and assistance to your team, particularly regarding adults with intellectual disabilities and children in care.
- Develop and manage individualized personal plans (IPPs) for assigned clients with disabilities and children in care.
- Read and provide feedback on the accuracy of information, practicality, and implementation of personal behavior support plans (PBSPs).
- Represent the organization at hearings of the Queensland Civil and Administrative Tribunal (QCAT) in matters associated with guardianship, financial administration, restrictive practices, and client issues.
- Develop care treatment regimens for clients with medical, mental health, and complex developmental and behavioral issues in association with other health professionals.
- Conduct regular supervision and training of care staff, preparing reports and documentation as required.
- Participate in an emergency after-hours contact roster to provide professional advice to care staff in managing critical incidents affecting clients and client care.
- Ensure compliance with human resource quality frameworks and NDIS practice standards and quality indicators.
- Maintain accurate records and ensure the appropriate use of client and organizational finances.
