Job Opportunity: Senior Care Management Professional

The role of a Senior Care Management Professional is multifaceted and demanding. It involves managing caseloads of clients with complex needs, providing professional care management advice and support to ensure that care practices meet relevant standards.

Key Responsibilities:

  1. Provide high-quality, professional care management advice and assistance to clients with disabilities and their carers.
  2. Develop and implement individualized care plans in collaboration with multidisciplinary teams.
  3. Represent the organization at hearings and tribunals, advocating for clients' rights and interests.
  4. Conduct regular supervision and training of care staff on-site and prepare comprehensive reports and documentation.
  5. Ensure compliance with quality standards and regulatory requirements.
  6. Engage with stakeholders to ensure service delivery meets individual client needs.

Requirements:

  • Tertiary qualifications in Psychology, Nursing, Human Services, Social Work, Special Education, or related fields.
  • Relevant experience or postgraduate qualifications are highly advantageous.
  • Comprehensive knowledge of legislation, policy, and practice in disability care, child protection, or aged care.
