About the Role

We are seeking a highly skilled and compassionate Care Navigator to join our team at Savvy Lawn Care Marvels LLC. As an AOD and Domestic / Family Violence (DFV) Care Navigator, you will play a critical role in providing expert case management and care coordination to individuals accessing our recovery services.

Your primary responsibility will be to deliver person-led, strength-focused support to our clients, working collaboratively with a dedicated multidisciplinary team to make a real impact in the local community.

Key Responsibilities
  • Provide Expert Case Management: Develop and implement comprehensive care plans, ensuring clients receive the best possible care and support.
  • Advocate for Change: Champion the reduction of stigma and discrimination related to AOD use, advocating for clients with government departments, courts, and other service providers.
  • Engage and Empower: Lead intake screenings, assessments, and care planning processes, supporting clients through evidence-based treatments and psychoeducation programs.
  • Develop Referral Pathways: Connect clients with community-based health and non-health services, engaging with families and significant others to support the wider community.
  • Collaborate and Innovate: Work with a dynamic team in regular clinical reviews and care coordination meetings, ensuring culturally appropriate treatments and fostering strong relationships with local service providers.
Requirements
  • Qualifications: TAFE Certificate IV in welfare or a related field.
  • Experience: Proven track record in supporting marginalized and vulnerable populations, including those affected by domestic/family violence and AOD issues.
  • Skills: Excellent communication, computer literacy, and the ability to work independently and as part of a team. Sound judgment in crisis situations and a commitment to safety are essential.
  • License: A valid driver's license.
