About the Position

We are seeking a Care Navigator to support the establishment of the Medicare Mental Health Centre Wagga Wagga. The successful candidate will be responsible for connecting consumers with services and supports in the community, facilitating person-centred and integrated care.

Key Responsibilities
  • Provide information sharing and education to consumers
  • Identify and address barriers to access
  • Facilitate warm referrals to ensure consumers are linked to the right service at the right time
  • Work collaboratively with the Medicare Mental Health Centre team and external service providers
Requirements
  • Tertiary qualification and/or extensive experience in a relevant discipline
  • Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems
  • Demonstrated understanding of supports and services available in the local and regional communities
  • Demonstrated ability to engage with consumers and their families or carers
