The Opportunity



We are seeking an experienced clinician to join our team as a Care Coordinator. As a key member of our Virtual Care Centre, you will play a vital role in managing acute patient care and enhancing the quality of life for our patients.

About the Role



As a Care Coordinator, you will be responsible for managing patient health requirements in a professional manner. You will work closely with our home visiting provider team to establish, monitor, and coordinate patients' treatment plans. Your clinical skills, care planning, and coordination skills will be essential in ensuring our patients receive best practice care.
