We are seeking an experienced Acute Care Manager to join our team in a virtual hospital setting. As a Care Coordinator, you will play a critical role in managing acute patient care and enhancing their quality of life.
Key Responsibilities- Manage patient's acute care health requirements in a professional manner
- Establish, monitor, and coordinate patients' treatment plans
- Utilize virtual technology and partner with our home visiting provider team
- Experienced clinician in nursing (RN)
- Acute assessment skills, care planning, coordination skills, and intuitive people skills
- Knowledgeable about acute healthcare practice and strong clinical reasoning and assessment skills
- Excellent remuneration including penalties for weekend shifts ($85,000 - $110,000 per year)
- Modern office location in Eastwood, Adelaide
