Job Title: Care Manager

We are seeking a dedicated and skilled Care Manager to join our team.

About the Role

The successful candidate will be responsible for:

  • Safeguarding participants' well-being and coordinating client care needs, including assessment, case management, and Care Plan development.
  • Employing professional skills and knowledge to assist clients in developing attainable goals and coordinating services that support these goals.

Responsibilities include:

  • Assisting the clinical management team with compliance documentation and comprehensive assessments.
  • Providing support and supervision of care staff.
  • Implementing training programs and delivering training to care staff.
  • Coordinating quality person-centred care according to client needs, service/care plans, and available funding.
  • Clinical in-home services include care plan assessments, medication, incontinence, wound, chronic disease, wound management, palliative care, and diabetes management.
  • Managing client budgets.
  • Providing prompt and solution-oriented service to clients.
  • Addressing and managing client complaints/feedback.
Requirements

To be successful in this role, you will need:

  • 2-3 years previous Case Management experience in NDIS or Home Care.
  • A comprehensive understanding of the NDIS framework, policies, and standards.
  • Demonstrated understanding of the needs of people living in the community.
  • Experience in coordinating a program for older people or people with a disability in the community and understanding of consumer-directed care.
  • Excellent documentation skills.
  • Computer literacy and competence in various apps.
  • Excellent communication skills with ability to liaise and consult with a wide range of clients, health professionals, and team members.
  • An own registered vehicle and current driver license.
  • NDISWC Clearance and Police Check.
  • Right to work in Australia.
