
Personalised Care Coordinator

We are seeking an experienced and motivated professional to coordinate the delivery of care services to our clients. Our clients are a mix of private, home care package and NDIS across all age groups.
This role is responsible for:
• Delivering outcomes and goals for our clients based on their care/NDIS plan
• Working collaboratively with our clients to coordinate supports and services to improve their quality of life
• Conducting client care assessments and care planning with a person centred care approach in alignment with goals/outcomes
• Understanding and managing government contractual requirements
• Supervising and managing care staff
• Scheduling appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements
• Coordinating the provision of quality person centred care according to the client's needs, service/care plans
• Building relationships with local stakeholders
• Having a positive attitude and ability to work in an ever changing environment
**Essential Requirements:**
• Experience within the aged care/disability/mental health in community setting
• Understanding of Aged care & Disability standards and consumer directed care
• First Aid, current CPR and National police check
• WWC (or willingness to obtain)
• Driver's licence - Australia
• High level of computer literacy
• Understanding of clinical governance and continuous improvement
• Ability to possess a 'can-do' positive attitude and work well in a team environment
**Desirable Skills:**
• EN/RN in community nursing 1-2 years with current AHPRA registration
• Cert III/Cert IV (Aged/Disability Care) minimum 4years experience
• Outgoing personality with high energy levels, good communication skills and personable telephone manner
• Ability to problem solve and think quickly under pressure
• Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisations
• Excellent customer service skills with exceptional verbal and written communication skills
• Sound knowledge and understanding of home care services and guidelines
• Knowledgeable in wound care, catheter care, peg feeding
• Sound knowledge and understanding of NDIS services and guidelines
• Excellent time management skills
• Previous experience with rostering is highly desirable
