Job Title: Support Coordinator
"> Job Overview. The role of a Support Coordinator is to work with NDIS participants to understand their plan and support options, assess funding and services available, and assist clients in understanding their rights and building confidence to make informed decisions.Key Responsibilities:

- Establish rapport and build strong working relationships with NDIS participants and their support network;">
- Assess the funding and services which are available to the clients and act on new referrals in a timely manner;">
- Assist clients, their families, carers and nominees to understand their NDIS plans, their rights and build their confidence and abilities to make informed decisions;">
- Work with the clients to conduct wellness plan, risk assessment and support plan to meet their NDIS goals;">
- Work with the clients, and their support network, to obtain quotes and assessments to meet the elements in the plan and provide maximum value;">
- Connect clients to a range of quality providers, community, mainstream and other services that provides dignified access and support to coach and support clients to meet their needs and achieve their set goals;">
- Collaborate with NDIS, mainstream, and existing or potential supports in family, carers and nominees to provide clients with options for linkages, referrals and supports for clients;">
- Manage complexity and crisis intervention;">
- Using a strength-based approach to work with clients to increase their skills and capacity;">
- Support clients to implement the plan and monitor their supports;">
- Regularly review and refine plans with clients and report outcomes to the NDIA through progress reports within the specified timeframes;">
Requirements:
">- Degree holder of community service, social work, mental health and other allied health professions preferred, ideally with registration with appropriate professional bodies;
">- Police Clearance/Check;
">- NDIS Worker Screening Check;
">- Working with Children Check;
">- Current First Aid and CPR certificate or willing to obtain;
">- Current driver's licence;
">- COVID-19 Vaccination Certificate;
">- Strong communication skills;
">- Demonstrated ability to work in a person-centred environment to build capacity, choice and control;
">- Experience in working with NDIS participants with complex support needs and in community networking preferred;
">- Ability to build and maintain rapport with a diverse range of NDIS participants and stakeholders, including people living with a disability, cultural diversity and language other than English;
">Desirable Qualifications:
">- Understanding and experience working with people with disabilities. Lived experience is highly regarded;
">- Experience in support coordination, local area coordination, case management, welfare work, mental health and community services;
">- Experience in working with autistic children and people with mental illnesses highly preferred;
">- Ability to advocate on behalf of clients and a commitment to their choice and control;
">- Understanding of National Disability Insurance Scheme Act 2013, the National Standards of Disability and relevant state-based legislation;
