About the Role

Southern Cross Support Services (SCSS) is a leading provider of NDIS services throughout Queensland, dedicated to empowering individuals with purpose, belonging, and security in their daily lives. Key Responsibilities
  • Ensure the effective management of allocated clients and their homes, overseeing all aspects of practice, process, and operations.
  • Support the implementation of NDIS Quality Standards and Human Rights, demonstrating best practice in all interactions.
  • Collaborate with Case Managers to ensure compliance with client profiles and support plans, including ISP, PBSP, and safety plans.
  • Develop and implement weekly client Activity Plans and meal plans, incorporating client input and preferences.
  • Lead the team, fostering a positive team spirit and ensuring a strong focus on client goals and outcomes.
  • Coordinate correspondence with external parties and the team, ensuring accurate and up-to-date information is readily available.
  • Ensure timely completion of required documentation by the team.
  • Contribute to stakeholder and guardian communication as required, in collaboration with Case Managers.
  • Plan and lead Core Team Meetings, and complete shadow shifts with new team members.
  • Conduct weekly internal audit checks to maintain high standards.
Requirements
  • Flexibility to work across a 7-day roster, including shift work, weekend, public holidays, night shifts, and overnight sleepovers.
  • Experience in a similar senior position (desired but not essential).
  • Ability to support across additional disability programs.
  • Attention to detail and time management skills for client documentation and shift reports.
  • Blue Card for Working with Children & NDIS Worker Screening (or Yellow Card).
  • Driver's licence and own vehicle with comprehensive insurance.
  • NDIS E-Learning modules.
