Housing and Wellbeing Service Coordinator

The Housing and Wellbeing Service Coordinator will work closely with individuals and families in partnership to empower them to make positive changes and address their identified housing and wellbeing needs.

This role involves extensive community outreach, engaging with individuals and families in crisis accommodation, street outreach and refuges, and building strong ongoing relationships.

  • Deliver high-quality support and interventions based on a client-centered approach.
  • Provide accurate and timely case management practices and record-keeping that meet operational requirements.
  • Pursue clients through assertive outreach strategies.
  • Facilitate client-led assessments and Care Plans to identify client goals.
  • Implement targeted interventions and family group meetings to achieve client goals.
