Advanced Practice Child Health Specialist

We are seeking an experienced and dedicated Nurse/Midwife/AHP to join our team as a Child Health Outreach Program Nurse/AHP.

The successful candidate will be responsible for working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.

This role has a strong clinical focus on outreach work, including liaison with internal and external stakeholders involved in the child and family's care.

About You
  • You hold current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA);
  • You have demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health;
  • You possess a working knowledge of Child Health and an understanding of current practices, theories and values;
  • You have an understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control, as well as knowledge of current issues affecting the lives of Aboriginal People;
