At Story Health, we are revolutionizing healthcare by leveraging cutting-edge virtualization technology to transform patient and clinician experiences. Our team is seeking a skilled RN Health Coach to join our dynamic organization.

This remote position requires facilitating patient support to optimize their medical therapy and serving as the liaison with healthcare providers.

Responsibilities
  • Deliver high-quality remote patient care, utilizing digital communication tools, including texting, phone calls, and telehealth platforms.
  • Provide telephone triage to patients experiencing acute symptoms.
  • Collaborate with patients and their caretakers to enhance engagement and help overcome barriers in their healthcare journey.
  • Guide patients through changing medical therapies using a clinician-designed protocol, ensuring seamless connection with clinicians when conditions change.
  • Act as the interface between patients and other healthcare stakeholders, including providers, clinics, pharmacies, etc.
  • Other responsibilities as required.
Required Skills and Qualifications
  • Active Registered Nurse license in a compact state.
  • Minimum 2 years of clinical experience in a specialty practice or acute care setting.
  • Bachelor's degree or equivalent required.
  • Exceptional written and verbal communication, as well as interpersonal skills, to interact effectively with patients, caregivers, and healthcare providers.
  • Strong analytical skills for problem-solving patient issues and preparing reports.
  • Highly developed customer service skills, with ability to identify problems and formulate solutions.
  • Able to multitask in a fast-paced, dynamic environment.
  • Passion for implementing technology to improve healthcare and willingness to quickly learn new software and tools.
  • Reliable home internet access.
