RN Health Coach

Story Health is an exciting new startup transforming healthcare through virtualization of patient and clinician experiences. Story Health is seeking a RN Health Coach to join our organization. You will facilitate patient support to optimize their medical therapy and serve as the liaison with the healthcare provider.

This is a remote position working Monday - Friday in the Eastern time zone (8 AM - 5 PM Eastern) or Central time zone (8 AM - 5 PM Central).

Responsibilities

- Provide remote patient care including coaching and education using digital communication tools (including texting, phone calls, and telehealth platforms)
- Provide telephone triage to patients with acute symptoms
- Work with patients and their caretakers to increase engagement and help them overcome barriers in their healthcare journey
- Guide patients with their changing medical therapies using a clinician-designed protocol. Connect patients back with their clinicians as needed when their condition changes
- Serve as the interface between the patients and others in the healthcare system, including providers, clinics, pharmacies, etc
- Other responsibilities as required

Key Qualifications

- Current active Registered Nurse license in a compact state
- 2+ years clinical experience in a specialty practice or acute care setting
- Bachelor’s or equivalent degree required
- Highly proficient written/verbal communication and interpersonal skills to interact with patients, caregivers, and healthcare providers
- Analytical skills required to problem solve patient issues and prepare reports
- Strong customer service skills, with ability to identify problems and formulate solutions
- Able to take on multiple tasks in a dynamic, fast-paced, and changing team environment.
- Passion for implementing technology to improve healthcare and able to quickly learn and use new software and tools
- High-speed home internet access
- Quiet home office space, free of distractions

Nice to Have

- Fluent in Spanish
- Experience working with patients in a virtual environment (via phone or telehealth platforms)
- Prior coaching or patient education experience

Salary range for this position is $65,000 - $74,000.
