About the Role

Careerone Partner Network is seeking a highly skilled and compassionate Disability Support Specialist to join our team. As a key member of our organization, you will play a vital role in providing comprehensive support services to individuals with disabilities.

Key Responsibilities
  • Clinical Assessment and Support
    • Conduct thorough assessments of clients' needs, including functional capacity, physical and mental health, and social and emotional well-being.
    • Develop and implement personalized support plans to meet clients' unique needs and goals.
  • Client Management and Support
    • Work collaboratively with clients, their families, and internal teams to deliver supportive care services.
    • Monitor clients' progress, identify areas for improvement, and make recommendations for service enhancements.
  • Quality and Compliance
    • Ensure accurate and up-to-date client records are maintained.
    • Participate in quality assurance activities and contribute to the ongoing improvement of services.
Requirements
  • Relevant qualifications in a disability or health field, particularly in a nursing setting.
  • Demonstrated clinical experience in working with people with complex clinical conditions and disabilities.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Current Australian Driver's license and willingness to travel.
