Job Opportunity:

We are seeking an enthusiastic and highly skilled person to fill the position of Disability Services Coordinator in our Disability Services Team.

About the Role:

Catering to the needs of individuals from diverse backgrounds, we provide a comprehensive range of social welfare services including residential aged care, home ageing, settlement and health, vocational and training, disability services, child care, and community-based services.

The successful candidate will develop positive relationships with customers, their families, and stakeholders. This includes intake referrals from various channels, providing responsive support coordination service, ensuring people with disabilities exercise choice and control when receiving supports and services.

Key Responsibilities:

  • Develop positive working relationships with customers and stakeholders
  • Intake referrals and create service bookings through the NDIS provider portal
  • Provide personal centered support coordination service
  • Support and assist individuals to access NDIS and other disability services
  • Responsible for responding to Request For Service (RFS) and claiming matters through the NDIS provider portal
  • Carry out general administrative duties and networking tasks
  • Comply with NDIS Code of Conduct and WHS Regulations and Guidelines

Requirements:

  • Good command of spoken and written English
  • Fluency in spoken and written Chinese (Cantonese and Mandarin)
  • Tertiary qualifications in Social work, Education, Leisure and Recreation, Welfare, Disability or related disciplines
  • Relevant knowledge and sound understanding of relevant legislation on NDIS and other community services
  • Excellent communication, interpersonal, time management, and organisational skills
  • Computer literate
  • Ability to work as a team and independently
  • Hold valid police check certificate
  • Willing to undergo NDIS Worker Screening Check prior to employment
  • Hold valid Driver's Licence and own a fully insured vehicle
  • Staying up-to-date with COVID-19 Vaccination

Preferred Qualifications:

  • Experience working in NDIS Support Coordination and Plan management
  • Willingsness to travel across different Sydney metropolitan areas to meet prospective customers
