Senior Care Coordinator Role

We are seeking a skilled professional to lead our care coordination process, delivering high-quality, compassionate care tailored to each client's unique needs.

This pivotal role involves overseeing the care coordination process, ensuring our clients receive exceptional support and guidance in managing their services.

  • You will work closely with a caseload of clients in our Self-Managed Plus (SM+) program, fostering meaningful relationships and working collaboratively to help individuals stay independent at home.
  • Effective communication is key in this role, as you will engage with clients to understand their needs and preferences, helping them plan services like personal care, cleaning, and allied health, while coordinating communication between clients, families, and healthcare providers.
  • Additionally, you will support clients to manage their budgets confidently, coordinate with internal teams and subcontracted providers, conduct regular check-ins to ensure everything is on track, and maintain accurate records of your work.
