We are seeking a compassionate and dedicated individual to join our team as a Senior Care Specialist.
Job Description:As a Senior Care Specialist, you will be responsible for conducting initial client intake and screening assessments. You will work closely with clients to develop and review individualized care plans that meet their unique needs.
Key responsibilities include:
- Conducting thorough initial screening assessments to identify client needs and develop personalized care plans
- Providing quality care to clients based on their individualized care plans
- Managing client information in CRM & CMS systems to ensure accurate and up-to-date records
- Engaging in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team
- Providing transportation and assistance with shopping and appointments
To be successful in this role, you will need:
- Certificate III Aged Care or Individual Support qualification (or equivalent)
- Current Australian Driver's Licence (Green Provisional minimum)
- Roadworthy and insured vehicle to use for work purposes
- Experience with technology including smartphones, tablets, laptops etc.
