Therapeutic Case Manager

This role involves providing culturally safe and trauma-informed case management to support clients in their recovery journey. The ideal candidate will have experience working with individuals and families affected by addiction, and possess strong communication and cultural awareness skills.
The successful applicant will be responsible for conducting assessments, maintaining accurate case notes, and collaborating with other professionals to deliver comprehensive care. They will also contribute to the development and implementation of programs aimed at promoting wellbeing and preventing relapse.
Key responsibilities include:
- Providing individualized case management services to clients
- Conducting risk assessments and developing safety plans
- Maintaining accurate and up-to-date case records
- Collaborating with multidisciplinary teams to deliver coordinated care
Required qualifications and skills include:
- Certificate IV in Community Services (AOD) or related qualification
- Experience in case management within AOD, therapeutic, or community settings
- Strong communication and cultural awareness skills
- Ability to work collaboratively as part of a team
- Flexibility and adaptability in a dynamic environment
