Job Overview

We are seeking a dedicated and experienced professional to join our team as a Case Manager.

Your Opportunity

This role involves providing case coordination with individuals and their families who are experiencing difficulties due to alcohol or other drug use.

Your Key Responsibilities

  • Complete assessments and intake processes for allocated individuals
  • Support individuals to create holistic and strength-based individualised support plans utilising SMART goals and referral to supplementary services as needed
  • Work collaboratively with staff to provide specialised support to address complex cases or emergency situations
  • Able to work until 8pm on three days a week
  • Actively build networks with specialist AOD and Mental Health services and participate in service meetings
  • Provide brief intervention and informal counselling based on motivational interviewing techniques
  • Facilitate therapeutic and educational groups, covering AOD, mental health, and wellbeing, with individuals engaged in the service
  • Provide AOD information and resources to individuals, families, internal staff, external service providers, and community, where possible

Requirements for Success

  • With a significant background in the AOD and/or child, youth & family sectors, you will be confident in your case management and assessment processes
  • A Degree or Diploma in Social Welfare/Social Science/Youth Work or a minimum of three years' experience in a similar role
  • Demonstrated experience providing outreach, case management, case coordination, assessment, counselling, and crisis intervention skills, including working with people experiencing alcohol and other drug use issues, mental health issues, mental health crisis, and working with intoxicated individuals
  • Experience and ability to work with at-risk individuals, including knowledge of child and adolescent development and child and youth safe practices
  • Well-developed communication and interpersonal skills, including report writing and data collection
  • Full Driver Licence with no restrictions
