  • The Opportunity

    We are currently seeking a skilled Case Coordinator to support our SA Community Care and My Home Hospital programs. As a Case Coordinator, you will play a central role in ensuring comprehensive client-centered management plans are achieved.

    • Key Responsibilities
      • Conduct initial phone assessments and care planning to support client rehabilitation and recovery.
      • Coordinate in-home services to ensure seamless care delivery.
      • Build and maintain effective relationships with healthcare professionals and stakeholders.
      • Monitor patient progress and update care plans as necessary.
      • Provide telephone support and education to clients and their families.
    • About the Role

      This is a permanent, part-time or full-time opportunity with a rotating 7-day roster. We are seeking RNs with minimum 2 years' experience and recent care coordination experience.
