Advanced Care Nurse

The role of a Hospital in the Home (HITH) Nurse Practitioner is to provide person-centered care at an advanced nursing practice level incorporating diagnostic and treatment interventions using high-level knowledge, skills, and autonomy to achieve people-focused quality care.
- Provide holistic integrated care in line with NSW Health Hospital in the Home PD2025_004.
- Work collaboratively as part of a multidisciplinary team to deliver comprehensive care to patients at home, residential aged care facilities, community settings, or the Hospital in the Home (HiTH) unit.
- Develop and implement individualized person-centered care plans based on evidence-based practice.
- Diagnose and treat complex health conditions using advanced clinical reasoning and decision-making skills.
- Collaborate with medical officers and other healthcare professionals to ensure seamless patient care transitions.
- Provide education and support to patients and their families to promote self-management and independence.
- Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia.
- OR Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) working towards Nurse Practitioner endorsement with 5 years post-graduate experience and at least 3 years in the relevant field.
- Demonstrated ability to work independently and in the absence of a Medical Practitioner to provide quality care.
- Exceptional clinical communication and interpersonal skills.
