About the Role

We are seeking a compassionate Home Care Support Specialist to provide exceptional care and support to our clients in their homes.

As a key member of our team, you will be responsible for delivering high-quality, person-centered care that prioritizes the dignity and well-being of our clients.

Your role will involve providing personal care and assisting clients with daily activities, following their individual care plans. You will work closely with Registered Nurses to deliver holistic care, including personal hygiene, toileting, nutrition, mobility, and more.

You will identify client care needs and collaborate with the client's care team to ensure excellent results. Effective communication is essential, as you will work with clients, their families, and team members to promote a positive and supportive environment.

You will also support clients with light housekeeping duties, such as laundry, cleaning, and meal preparation, to promote independence and confidence.

Key responsibilities include:

  • Providing personal care and assisting clients with daily activities
  • Working closely with Registered Nurses to deliver holistic care
  • Identifying client care needs and collaborating with the client's care team
  • Effective communication with clients, their families, and team members
  • Supporting clients with light housekeeping duties to promote independence
Requirements and Qualifications
  • Cert III in Aged Care/Disability (or equivalent)
  • Current Senior First Aid certification
  • A roadworthy vehicle with a current driver's license
  • NDIS Screening or willingness to obtain it
  • Strong expertise in mental health care
  • Flexibility with shifts and willingness to travel
  • Strong communication and interpersonal skills with empathy for elderly clients
Preferred Skills
