Home Care Nurse

We are seeking a compassionate and skilled Home Care Nurse to deliver high-quality in-home care and assistance to our diverse client base.
This is a unique opportunity to make a positive impact on the lives of individuals in their own homes. As a Home Care Nurse, you will be responsible for providing personalized care services that cater to the specific needs and preferences of each client.
Key Responsibilities:- Assess clients' needs and develop personalized care plans that meet their goals and preferences.
- Deliver high-quality care services that promote independence, dignity, and well-being.
- Communicate effectively with clients, family members, and other healthcare professionals to ensure seamless care delivery.
- Monitor and review care plans regularly to ensure they remain relevant and effective.
- Demonstrated experience in home care nursing, preferably in a community setting.
- Knowledge of Government-funded HCP programs and the Aged Care Act.
- Advanced communication and interpersonal skills, with the ability to work autonomously or within a team environment.
- Reliable transportation and a valid driver's license.
- Australian National Police check and First Aid/CPR certification.
- Professional Indemnity Insurance and commitment to ongoing professional development.
- Passion for delivering high-quality, client-focused care in a community setting.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Excellent time management and organization skills.
