Job Overview

We are seeking a skilled professional to provide support and guidance to individuals living with mental illness.

About the Role

This part-time position is located in Queensland, Australia, and involves working within the Transitional Recovery Program (TRP) to help individuals increase their independence, maintain their wellness, and improve their quality of life.

Key Responsibilities
  • Provide case management, group work, and community development interventions to individuals living with disability and/or mental illness.
  • Develop and implement client individual support plans using Evidence-Based Practice.
  • Conduct regular planning reviews and ensure plans are monitored and progressed.
  • Maintain a positive environment that empowers service users and promotes independence.
  • Communicate effectively with stakeholders.
  • Complete and store documentation in a confidential manner.
Requirements
  • Tertiary qualifications in a relevant field or studying towards such a qualification.
  • Relevant experience in the Community Services sector or transferable skills from a relevant industry.
  • Effective interpersonal and communication skills.
  • Accurate decision-making skills.
  • Ability to work in a fast-paced environment.
  • Basic understanding of Outlook, Excel, Word, and mobile apps.
  • Current Working with Children Check (Blue Card), NDIS Worker Screening Clearance, and Australian Driver's Licence.
  • Reliable vehicle for transporting clients as required.
