Job Description

This role involves conducting post-discharge follow-up to assess patient progress, address concerns and prevent readmissions. You will also generate reports on key performance indicators, client outcomes and other relevant metrics.

 

  1. Participate in continuous quality improvement activities.
  2. Conduct post-discharge follow-up to assess patient progress, address concerns and prevent readmissions.
  3. Generate reports on key performance indicators, client outcomes and other relevant metrics.
  4. Analyze data to identify trends, areas for improvement and opportunities for optimization.
  5. Manage the referral process, including receiving and processing referrals from healthcare professionals or agencies.
  6. Ensure timely communication and coordination with referring entities.

 

Required Skills and Qualifications

The ideal candidate should have experience in case management, preferably in the aged care sector. A Bachelor's degree in Nursing is preferred, but not essential. The candidate should also possess strong organizational, communication and interpersonal skills.
