Care Partner Support Officer Role Overview

The Care Partner Support Officer plays a vital role within our Community Services team, coordinating care and services for clients residing in their homes. This position requires strong communication and problem-solving skills, as well as the ability to work independently and collaboratively as part of a team.

  • Key Responsibilities:
  • Establish rapport with clients and their representatives
  • Manage a dedicated caseload of clients, meeting all monthly, quarterly, and annual review requirements
  • Review and recommend person-centered care plans that are human rights based, trauma aware, healing informed, and reflect the reablement approach
  • Identify eligibility for funding: palliative care, assistive technology, home modifications, restorative care
  • Seek clinical advisory support to ensure the clinical needs of the client are met and regularly reviewed
  • Identify changes in health and wellbeing by reviewing care needs and services
  • Home Visits: Meet and Greet, six-monthly monitoring, annual reassessment, and other visits as required
  • Complete assessments of the client's home environment and respond to any identified hazards
  • Maintain current and accurate client records and case notes
  • Identify vulnerable clients and ensure their care plan reflects needs associated with vulnerability
  • Maintain professional boundaries with clients and their representatives
About You:

This role is ideal for individuals who possess a bachelor's degree or equivalent qualification in a health-related field or allied health. A qualification in Community Services or an equivalent qualification is also preferred.

  • Business Acumen and budget handling skills
  • Experience working in Community Aged Care, including knowledge of issues facing older people or people with disabilities and their carers
  • Demonstrated understanding of Strengthened Aged Care Standards and best practice in community aged care
  • Excellent communication, problem-solving, influencing, and negotiation skills
  • Strong interpersonal skills and ability to prioritize tasks and adapt to changing demands
  • Able to work within the scope of your role while ensuring all KPI requirements are met
  • Well-developed PC skills and experience working with a culturally diverse workforce
