Job Opportunity: Care Coordinator

We are seeking a dedicated and enthusiastic Full-Time Care Coordinator to join our dynamic team.

About the Role:

The successful candidate will be responsible for building strong relationships with clients and care workers, ensuring seamless communication and exceptional service. A key aspect of this role is managing a caseload of up to 35 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.

Main Responsibilities:
  • Build strong relationships with clients and care workers.
  • Manage client caseloads and coordinate care plans.
  • Conduct home visits to introduce clients to our care workers.
  • Utilize computer skills for efficient record-keeping and communication.
Requirements:
  • A minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker.
  • A valid QLD Driver's License.
  • A First Aid & CPR Certificate.
  • A Yellow Card/NDIS Worker Screening Clearance.
