Job Title: Disability Support Worker

As a Disability Support Worker, you will play a vital role in supporting individuals with disabilities to achieve their goals and aspirations. This is an exciting opportunity to make a positive impact on people's lives.

The successful candidate will provide person-centred care, contributing to the development of Customer support plans and providing support with personal care, living skills, transport, social activities, medication, and other needs as required.

To be successful in this role, you will need Cert 3 or 4 in disability, Individual support, Aged Care, Nursing, and experience in the disability, aged care, or nursing sector.

You will also have strong communication, problem-solving, and conflict resolution skills, as well as excellent time management skills and attention to detail.

Key Responsibilities:

  • Provide person-centred care to help customers achieve their goals and aspirations.
  • Contribute information for the development of Customer support plans.
  • Provide support to customers with personal care, living skills, transport, social activities, medication, and other needs as required.
  • Keep customer records up to date on a daily basis.
  • Develop and maintain professional relationships and networks with customers, families, caregivers, and key stakeholders.

Requirements:

  • Cert 3 or 4 in disability, Individual support, Aged Care, Nursing.
  • Experience in disability, aged care, or nursing sector.
