Pathways to Good Health Navigator

The successful applicant will have a strong understanding of child protection and out-of-home care systems, as well as excellent communication and partnership building skills.

The Pathways to Good Health Navigator will work closely with health practitioners, VACCA, Dept Communities and Justice, carers, and service providers across the region to ensure that children receive timely, culturally appropriate health assessments, referrals, and wrap-around care.

This role is a critical bridge between families, carers, and service providers, empowering children and their families to navigate complex health systems with confidence and trust.

Key Responsibilities:

  • Support the implementation of quality, culturally-sensitive care plans for children and young people
  • Collaborate with Pathways to Good Health team members, VACCA, Dept Communities and Justice, carers, and service providers
  • Advocate for families to ensure respectful and inclusive service delivery

Requirements:

  • Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice, or willingness to obtain qualification
  • Strong understanding of child protection and out-of-home care systems
  • Proven ability to build trust, advocate for families, and coordinate care plans
