Key Responsibilities:- Develop effective referral pathways for support services
- Build key stakeholder relationships and maintain open communication channels
- Collaborate with medical and non-medical professionals to ensure comprehensive support for clients and their families
This position requires a deep understanding of the health and wellbeing needs of Aboriginal and Torres Strait Islander peoples, particularly in relation to cancer care. The successful candidate will have experience working in community health, cancer services, or a related health or social service environment.
About the Team:The Cancer Program provides practical day-to-day support for Aboriginal and Torres Strait Islander people/families and community in navigating the health and community support systems involved in the cancer journey.
