Key Responsibilities:

  • Plan, manage, and review client goals to ensure optimal care outcomes.
  • Actively communicate with team members and clients to develop comprehensive care plans.
  • Formalize agreements with clients and families regarding care services and costs.
  • Accurately document interactions with clients, families, or stakeholders.

 

Requirements:

  • Prior experience working with seniors or aged care is essential.
  • Familiarity with Home Care Packages is highly desirable.
  • Proven time management skills and ability to multitask under pressure.
  • Strong written and verbal communication skills.
  • A valid Australian Driver's License.
  • A qualification in a health-related discipline is highly regarded.
