About Us:
We are seeking a Senior Care Coordinator to join our team based in Elizabeth Lodge, Rushcutters Bay on a permanent full-time basis.
The role of the Senior Care Coordinator is to oversee and coordinate personalized care plans for clients, ensuring their health and wellbeing align with medical requirements and individual preferences.
Key Responsibilities include:
- Establishing comprehensive communication between the multidisciplinary team within the facility and other relevant staff.
- Working closely with residents' families to ensure they are fully informed about clinical issues relating to their relative.
- Contributing to the effective functioning of the team through mentoring staff and sharing clinical knowledge and skills.
Requirements:
- Current nursing registration with AHPRA
- Minimum 3 years experience as Care Manager or senior Registered Nurse within Aged care
- A 'hands-on' approach to ensuring quality clinical care
- The ability to supervise, delegate and work as part of a team
- A commitment to delivering high-quality patient care
- Demonstrated knowledge of aged care accreditation standards
- Post graduate study in relevant discipline/s (geriatrics, dementia, palliative care) highly desirable
