Fostering Young Lives Case Manager

About the Job- Build strong relationships with clients and their families to ensure they receive comprehensive support.
- Conduct thorough assessments to develop, implement, and review client plans.
- Coordinate with various service providers to ensure access to wrap-around services.
- Develop and maintain client relationships to deliver high-quality support.
- Conduct risk assessments and develop strategies to mitigate risks.
- Work collaboratively with other agencies to achieve optimal outcomes.
- Experience working with vulnerable populations.
- Strong assessment and planning skills.
- Effective communication and interpersonal skills.
