Care Coordinator

About the Role
We are seeking a skilled Care Coordinator to join our team and play a vital role in supporting elderly individuals in the community to live independently and with dignity in their own homes.
In this role, your primary responsibility will be to manage a caseload of community-based elderly individuals with Home Care Packages. You will assess their needs, develop personalized care plans, and manage budgets in accordance with program guidelines to ensure each customer's unique requirements are met.
You will onboard new customers and proactively manage existing ones with a person-centered approach. Collaboration with our Clinical, Scheduling, and Service Delivery teams will be crucial to delivering comprehensive care.
In this permanent full-time role, you will have the flexibility to manage your own schedule each week and work from home when you're not on the road visiting customers. While most of your caseload will be located in Hobart and the surrounding suburbs, occasional travel to Northern Tasmania will be required.
About You
To be successful in this role, you will have:
- Previous experience in community-based care coordination (desirable)
- Experience with Home Care Packages (desirable)
- Current driver's license
- Reliable vehicle with minimum Third Party insurance
- Strong communication skills, both verbal and written
- Demonstrated empathy and compassion
- Excellent problem-solving abilities and critical thinking skills
- Good organizational and time management skills
- Cultural competence and the ability to work with diverse populations
- Advocacy skills to ensure clients' needs are met
- Collaboration and teamwork skills to work effectively with interdisciplinary teams
