About the Role
As a Care Coordinator, you will play a vital role in supporting elderly individuals to live independently and with dignity in their own homes. Your primary responsibility will be to manage a caseload of community-based elderly individuals with Home Care Packages.

You'll also onboard new customers and proactively manage existing ones with a person-centred approach. Collaboration with our Clinical, Scheduling and Service Delivery teams will be crucial to delivering comprehensive care.
This is a permanent full-time role that offers flexibility, allowing you to manage your own schedule each week and work from home when you're not on the road visiting customers. While most of your caseload will be located in Hobart and the surrounding suburbs, occasional travel to Northern Tasmania may be required.
