- Conduct comprehensive assessments to identify individual needs and develop personalized plans to support people in their homes and communities.
- Develop and implement Behaviour Support Plans using a positive behaviour support framework, with a focus on eliminating restrictive practices.
- Collaborate with a small, cohesive team to provide high-quality support and services.
- Mentor and support junior clinicians to ensure the development of their skills and knowledge.
